Fall Middle School Sports Fall Information

The 2011-12 fall Middle School sports program will be co-sponsored by the Montgomery Township Board of Education and various township and/or parent-led organizations.  The sports programs will be offered on a fee-for-service basis through the sponsoring organization.  The Board of education will be funding a portion of the program as well.  Please contact the person listed below if you have specific questions regarding any programs offered.

If your son or daughter will be signing up for a middle school sports team for this spring, you will need to do the following:

• Pick up the following forms at your schools nurses office starting Tuesday May 31st.  The forms may also be downloaded from the districts web site.  Click on the departments tab, then athletics, then athletic forms.

·         Health History Questionnaire – Part A

·         Physical Evaluation Form – Part B

• To sign-up for lower and upper middle school cross country & field hockey, you will need to register with the Montgomery Recreation Department as well as attend the team meetings on June 14th or June 15th.  Registrations for those sports will begin on July 1st at the Otto Kaufman Center on Skillman Road.  Students going into grade 5 next year, only need to sign up at the recreation department.
