COVID-19 Information, Guidelines and Forms
The governing body for high school sports in New Jersey (NJSIAA) has released regulations for summer workouts to begin. Student-athletes will be notified by their coach if their program will be conductig summer workouts under the NJSIAA Guidleines. Specific information regarding Phase 1 & 2 guidelines are posted below. In addition, the NJSIAA has adopted a Delayed Model for the start of Fall athletics. Please refer below to the attached links.
These sport specific questionnaires are Google Forms to be completed by the parents of the student-athlete before they can be cleared to participate in any summer workout.
If any of the questions on this questionnaire are answered "Yes", a clearance letter from your physician is required. Physician clearance for Covid should include the following:
- Date of diagnosis/exposure
- Was the student asymptomatic or symptomatic
- Was the student hospitalized
- Include documentation that the patient is back to baseline and may participate in their respective sport or activity
**Only a Doctor of Osteopathic Medicine (DO) or a Doctor of Medicine (MD) can clear athletes for Summer workouts concerning COVID-19 signs, symptoms and history. Pre-Existing Condition Info:
Students who have a pre-existing medical condition and/or are immunocompromised shall provide clearance from their physician before he or she will be permitted to participate in summer workouts. A list of pre-existing conditions from the CDC can be found here
**This Form must be printed and completed by the student-athlete prior to arriving to his or her scheduled workout**
