2023-24 Winter Sports Registration Information
**Please note: The Montgomery Township School District will now be using Genesis (parent portal) for all sport registration forms. In addition, all health-related forms—such as physicals, health history updates and, if applicable, allergy action plans—must be physically turned into the nurse or (for MHS Students only) the confidential medical lockbox. We are no longer allowed to accept these forms electronically.**
Registration for winter sports will be starting on Monday, October 2nd. All winter programs will adhere to the start dates, guidelines, and protocols set by the NJSIAA, NJ DOH, and the Montgomery Township School District and are subject to change.
MHS
Winter Sports Offered: Basketball (boys and girls), Winter Track (boys and girls), Fencing (boys and girls), Swimming (boys and girls), Diving (boys and girls), Ice Hockey (co-op with Hopewell Valley High School), and Wrestling
Start Dates: Monday, November 20th for Basketball, Wrestling, Swimming, Diving, and Ice Hockey
Monday, November 27th for Winter Track and Fencing
*please note that Cheer and Dance are continuing from the Fall season

In order for your child to be cleared to participate on a sports team, the 3-step process detailed below must be completed.
Step 1 – Print & Submit Medical Forms:
Winter Sports sign-ups will begin the week of October 2nd. Please visit the Athletic Department website or download the appropriate form below (Physical or Health History Update) depending on what your child needs based on their last sports physical date.
All Winter team managers and statisticians will need to complete Step 1 and ONLY a Health History Update.
All Health forms are due to the MHS Nurse by Monday, November 6th and the UMS Nurse by Monday, October 30th.
All Physical Evaluation Forms must be reviewed by the school physician per NJ state law in order for your child to be cleared to participate. Please submit your forms to the nurse by the dates listed above to allow enough time for review and processing. Please do not wait until the due date to hand in your forms. Forms must be submitted directly to the school nurse or placed in the confidential medical lock box (MHS). Forms are not to be handed into the main office, emailed, faxed or mailed to the school at any time. The health office will deliver the forms to the school physician for review.
A new Asthma and or Allergy Action Plan Form is required once per year ONLY if your child’s physician has prescribed an EpiPen for your child’s allergy or an inhaler is prescribed for asthma. The allergy or asthma action plan must be renewed by your physician each school year. Please note that your child will not be allowed to participate in athletics without a completed and updated Asthma or Allergy Action Plan on file in the nurse’s office. Step 2 - NEW THIS YEAR: Register Online Through The Parent Portal (Genesis):
The Montgomery Athletic Department is happy to announce that we will now be using the Parent Portal (Genesis) for Athletic Registration. You will use the same exact username and password that you currently have to access the Athletic Registration acknowledgments and documents. Please note that you still must register for a team each sports season (Fall, Winter, Spring). Failure to register each season through the Parent Portal will prevent your child from being cleared to participate. The Parent Portal does not include the submission of the Physical Evaluation Form, the Health History Update Form or the Allergy Action Plan.
Genesis sports registration for the 2023 Winter Season will open on Monday, October 2nd, but you can turn in your 1.) Physical Form, 2.) Health History Update, and 3.) Allergy Action Plan (if applicable) to the nurse ASAP.
**REMINDER: Please note that our school physician is now requiring that all health-related forms be physically turned into the nurse and not uploaded electronically. We are no longer allowed to accept these forms electronically, so any form turned in electronically will not be accepted.
Parent Portal (Genesis) Registration Process:
A parent/guardian must register by going to the Parent Portal Link below:
Follow these steps:
- Login to Parent Portal
- Click on the FORMS TAB
- Next click on Winter Sports Participation Form
- Carefully scroll through the questions and print out any necessary documents
- Finally, click on UPDATE ANSWERS when completed
Step 3 - ImPACT Baseline Test (MHS Sports only):
The online ImPACT baseline test is required once a year during your freshman & junior years before the athlete’s first sport. If you are going to be in 9th or 11th grade or you are new to Montgomery High School, you will need to take the baseline test. If you are currently being treated by a doctor for a concussion, do not take the baseline test and see one of the Athletic Trainers ASAP.
